Program HH

Eligibility and Application

To be eligible for services through Program HH, you must:

  • Be HIV positive
  • Live in Minnesota, and
  • Have income at or under 400% of the Federal Poverty Guidelines ($5,020 per month for an individual).

You may be enrolled in Program HH Basic Services and Medical Assistance (MA) or Medical Assistance for Employed Persons with Disabilities (MA-EPD) at the same time.

Once enrolled in Program HH, you will need to report changes in income, assets, or household composition as soon as they happen by submitting an annual renewal form or calling 1-651-431-2398.
